Service Agreement Terms and Conditions

In retaining us to handle your medical license application process, you agree to the following:

• It is your responsibility to pay all fees collected by the medical board(s), credential holders, and entities that collect fees for information required to perform the process of obtaining a medical license.
• Processing timeframe of licensure can vary, and HealthPro Licensing Services will not guarantee a specific timeframe. Our recommendation is that you avoid making any professional and personal commitments until your license has been issued.
• As HealthPro Licensing Services manages your license application, we are not responsible for changes made by the medical boards that may affect your ability to be licensed, or that may require you to meet additional requirements.
• For the successful management of your license application, you have provided HealthPro Licensing Services with accurate and complete information. If any medical board(s) take action due to your omissions or inaccuracies, HPLS is not responsible for fines, denials, or any other consequences.

Document Reproduction

I acknowledge an additional fee will be required for document reproduction if an application document is lost or rendered outdated due to a delay on my (the client’s) part.

Data Omission

In the event I omit information required to perform the services, I agree to pay additional fees for application amendments or subsequent documentation requests. In addition, if I omit any pertinent information from the documents, whether intentionally or otherwise, it might reflect in the documents you prepare and could result in adverse actions from medical boards or other entities. If such a result occurs, I accept full responsibility for it.

International Credentials

I acknowledge your services will not include collecting records held or maintained by persons that are located outside the United States of America. If any licensing board requires records from persons that are located outside the United States of America, I will obtain those records.

Refund Policy

If the licensure application is cancelled for any reason before a license is issued, a partial refund of service fees paid are outlined below:

Tasks Completed:	Refund:
Step 1 (Research)	Up to 50% of service fee
Step 2 (Application preparation)	Up to 20% of service fee
Step 3 (Credentials verification)	No refund available

I acknowledge HealthPro Licensing Services may terminate our Service Agreement at any time due to any unprofessional or unethical conduct or if the file becomes dormant. In such a case, the refund policy outlined above will apply.
